This article explains how to generate and send ACA Consent to Contact forms from Individual Records in AMS+ by email or SMS text message.

Requirements
- The Electronic ACA Consent to Contact Forms setting must be enabled in My Agency Settings. Learn more.
- You must be logged into AMS+ as an Agency User or Agent CRM User.
- You must be assigned to a Security Group with the Individual Visible and Edit permissions.
- If you're an Agent CRM User, you need access to Individual Records assigned to your Accessible Agent, and optionally unassigned records.
- You must have added your phone number and NPN to your My Account profile settings. Learn more.
How to email ACA Consent to Contact forms
- Go to the desired Individual Record, and do one of the following:

Select the Sales link at the top and choose ACA Forms from the dropdown menu, or
<> - Go to the Consent tab and select the Create New button, then select ACA Forms.

- Choose the form language: English or Spanish.
This determines which email template is used (English or Spanish).
- In the Form field dropdown, select the ACA Consent option.
- Select a user in the Agent/User field dropdown.
- Enter or select a date from the date picker.
- Select the Send ACA Email button when ready.
AMS+ will email a unique ACA Consent to Contact form link to the record's primary email address. The recipient or their authorized representative can sign and submit the form.
You can also send the form by text.
How to text ACA Consent to Contact forms
ACA Consent to Contact forms are emailed by default. If the record includes a valid cell phone number, you can also send the form via SMS text message.
- Go to the Consent tab of the desired Individual Record.
- Locate an ACA Consent to Contact form with the status "Requested".
- Select the Text button to open the Send Text Message pop-up.
- Review the pre-filled message, edit if needed, and tap the Send Message button.
The message will include the ACA form link and the required compliance language.
Legacy components
The following content applies only to accounts still using Sales Records. If you're unsure whether this applies to you, check your subscription package on the Subscription page. Accounts on Enhanced Records will show a package type that includes "+ Enhanced Records" (e.g., Accelerate + Enhanced Records). If your package does not include "+ Enhanced Records," this section applies to you.

How to email an ACA Consent to Contact form from a Sales Record
- Go to a Sales Record and select the Generate ACA Forms button.
- Choose the form language: English or Spanish.

- Select a user in the Agent field dropdown.
- Enter or select a date from the date picker.
- Select the SUBMIT button.

Sales Record permissions
To email or text an ACA Consent to Contact form from a Sales Record, you must have user account Sales permissions to View and Edit Sales Records. Learn more.
