This article explains how to monitor the status of ACA Consent to Contact and Attestation forms in AMS+.

Requirements
- You must be assigned to a Security Group with the Sales Enablement Visible permission.
- You must be logged into AMS+ as an Agency User or Agent CRM User.
- To view requested and completed ACA Consent to Contact or Attestation forms:
- For Sales Records:
- You must also have user account Sales permissions to View Sales Records. Learn more.
- For Individual Records:
- Your assigned Security Group must also have the Individual Visible permission.
- If you’re an Agent CRM User, you need access to Individual Records assigned to your Accessible Agent, and optionally unassigned records.
- Your assigned Security Group must also have the Individual Visible permission.
- For Sales Records:
- To re-send a requested ACA Consent to Contact or Attestation form:
- For Sales Records:
- You must also have user account Sales permissions to View and Edit Sales Records. Learn more.
- For Individual Records:
- Your assigned Security Group must also have the Individual Visible and Edit permissions.
- If you’re an Agent CRM User, you need access to Individual Records assigned to your Accessible Agent, and optionally unassigned records.
- Your assigned Security Group must also have the Individual Visible and Edit permissions.

How to check ACA form status
You can check the status of sent ACA Consent to Contact and Attestation forms from the Sales Record or Individual Record Consent tab.
- The status will automatically update once the client/client signs and submits the form.
- If the client/lead has not received or signed the form by the appointment date, you can resend the ACA form via email or text message.
- If you are meeting the client/lead in person, select the 'OPEN FOR SIGNATURE' button to open the ACA form’s PDF and have them sign it on the spot.
- After signing, the ACA form’s status will update to "Completed", and you’ll be able to download the signed PDF
