This article explains how to monitor the status of ACA Consent to Contact and Attestation forms in AMS+.

Requirements
- You must be logged into AMS+ as an Agency User or Agent CRM User.
- To view requested and completed ACA forms:
- You must be assigned to a Security Group with the Individual Visible permission.
- If you're an Agent CRM User, you need access to Individual Records assigned to your Accessible Agent, and optionally unassigned records.
- You must be assigned to a Security Group with the Individual Visible permission.
- To re-send a requested ACA form:
- You must be assigned to a Security Group with the Individual Visible and Edit permissions.
- If you're an Agent CRM User, you need access to Individual Records assigned to your Accessible Agent, and optionally unassigned records.
- You must be assigned to a Security Group with the Individual Visible and Edit permissions.
How to check ACA form status
You can check the status of sent ACA Consent to Contact and Attestation forms from the Individual Record's Consent tab.
- The status will automatically update once the client signs and submits the form.
- If the client has not received or signed the form by the appointment date, you can resend the ACA form via email or text message.
- If you are meeting the client in person, select the OPEN FOR SIGNATURE button to open the ACA form's PDF and have them sign it on the spot.
- After signing, the ACA form's status will update to "Completed", and you'll be able to download the signed PDF.

The following content applies only to accounts still using Sales Records. If you're unsure whether this applies to you, check your subscription package on the Subscription page. Accounts on Enhanced Records will show a package type that includes "+ Enhanced Records" (e.g., Accelerate + Enhanced Records). If your package does not include "+ Enhanced Records," this section applies to you.

Sales Record ACA form status
You can also check the status of sent ACA forms from a Sales Record's Consent tab. The status behavior is the same as for Individual Records.
Sales Record permissions
To view ACA forms on Sales Records, you must have user account Sales permissions to View Sales Records. To resend ACA forms from Sales Records, you also need Edit permissions. Learn more.
Sales Enablement Visible permission
To view and manage ACA forms for Sales Records, you must be assigned to a Security Group with the Sales Enablement Visible permission.
