This article explains how to generate and send Scope of Appointment (SOA) forms from Individual Records and Sales Records in AMS+ by email or SMS text message.

Requirements
- The Electronic Scope of Appointments setting must be enabled in My Agency Settings. Learn more.
- You must be logged into AMS+ as an Agency User or Agent CRM User.
- You must be assigned to a Security Group with the Individual Visible and Edit permissions.
- If you’re an Agent CRM User, you need access to Individual Records assigned to your Accessible Agent, and optionally unassigned records.
- You must have user account Sales permissions to View and Edit Sales Records. Learn more.
- You must have added your phone number, signature, and sales rep ID to your My Account profile settings. Learn more.
How to email an SOA form
From a Sales Record
- Go to the desired Sales Record and choose Generate SOA.
- Choose the form language: English or Spanish.
This determines which email template is used (English or Spanish).
- Fill in the following information: agent details. method of contact, plans to be represented, and appointment date.
- Choose SUBMIT.
AMS+ will email a unique SOA confirmation form link to the record's email address. This form includes all CMS-required Medicare options and definitions for the appointment. The lead or their authorized representative can complete and submit it electronically.
By default, SOAs are sent via email, but you also have the option to send them by text.
From an Individual Record
- Go to the desired Individual Record, and do one of the following:
- Select the Sales link at the top and choose Scope of Appointment from the dropdown menu, or
- Go to the Consent tab and select the Create New button, then select Scope of Appointment.- Choose the form language: English or Spanish.
This determines which email template is used (English or Spanish).
- Fill in the following information: agent details, method of contact, plans to be represented, and appointment date.
- Choose Send SOA Email.
AMS+ will email a unique SOA confirmation form link to the Individual Record’s primary email address.
As with Sales Records, you can also send the form by text.
How to text an SOA form
SOA forms are emailed by default. If the record includes a valid cell phone number, you can also send the form via SMS text message.
- Go to the Consent tab of the desired Individual Record or Sales Record.
- Locate an SOA with the status "Requested".
- Select the Text button to open the Send Text Message pop-up.
- Sales Record:
- Individual Record:- Review the pre-filled message, edit if needed, and choose Send Message.
The message will include the SOA form link and required compliance language.
