The Centers for Medicare & Medicaid Services (CMS) mandates that life and health insurance brokers obtain a “consent to contact” before assisting with Marketplace plans. In essence, brokers must have permission from individuals, employers, or employees before helping with tasks like searching for applications, aiding in coverage application, enrolling in qualified health plans, or updating coverage details. An attestation form confirms that the information provided on a consumer’s ACA application is correct and true before it is submitted.

How to activate consent and attestation forms
You must belong to a Security Group with the ‘Manage User’ permission to activate ACA Consent and Attestation Forms.
- Go to Settings.
- Find the Sales > Optional Features setting, and select the 'Edit' button.
- Scroll down until you see the ‘ACA Consent’ section and make sure the 'Enable Electronic ACA Consent Forms’ checkbox is checked.
- This checkbox must be checked if you want to use ACA Consent Forms and/or ACA Attestation Forms.
- By default, the Consent Form Signature Method will be “Typed In” but you can change it to “Signature pad”.
- Assign English and Spanish email templates.
- When generating an ACA Consent Form, the form link will be sent via email. Your English and Spanish email options are configured in these settings. We provide default email templates, but you can also create, customize, and use your own. Learn how to create email templates.
Make sure the email templates contain the snippet #ACA_CONSENT_LINK# for the user to click on! Your email templates may also contain the snippet #ACA_DOCUMENT_TITLE# for the ACA Document Title (e.g. ACA Consent, ACA Attestation). - (Recommended) Enter an English Success URL and/or Spanish Success URL.

When an ACA Consent Form is completed, the form can redirect to a landing page or a custom URL of your own if you want to inform the respondent that their submission has been received. Learn how to create a landing page.
- When ready, select the “Submit Settings” button to save your changes.
- Now you’ve activated and enabled the ability for Agents to send consent forms to leads.
How to add your signature for consent and attestation forms
If ACA Consent Forms are enabled, you will need to add their sales rep contact details and signature to your user account. This information is necessary for generating consent and attestation forms.
To add your sales rep contact details and signature:
- Go to My Account.
- Navigate to the ‘SOA / ACA Consent’ tab and add your phone number and NPN number to the SOA Contact Info section.
- Save.
- Scroll down and upload your signature or sign with the signature pad.
- Add your Sales Rep ID.
