Manually creating Rx Collect forms for Individual and Sales Records
Sales Record
- Go to the beneficiary’s Sales Record in AMS+.
- At the top of the record, select the “Rx Collect” button.
- This automatically sends an email to the email address on the record’s Detail tab. The record must have an email address for you to send the Rx Collect form.
Individual Record
- Go to the beneficiary’s Individual Record in AMS+.
- In the top right corner of the record, select the 'Sales' link, then choose 'Rx Collect' from the dropdown menu.
- This automatically sends an email to the email address on the record’s Detail tab. The record must have an email address for you to send the Rx Collect form.
Note: The form link will be valid for up to 48 hours. After 48 hours have elapsed, the form link will expire, and you will need to send a new link to the beneficiary.
Mass sending Rx Collect forms with Sales Workflows
You can mass send Rx Collect forms using Sales Workflows, but this option is only available for Sales Records and certain AMS+ subscription packages. If you need to generate Rx Collect forms for Individual Records, you must do so manually, one at a time.
How it works
- Create a Sales Workflow that triggers when the specified criteria are met.
- When triggered, the workflow automatically generates an Rx Collect form for all qualifying Sales Records.
- Once set up, any Sales Record that meets the workflow criteria will automatically receive a form.
Tip for Individual Records
If you need to generate Rx Collect forms for Individual Records, consider creating Sales Records for them first. Use our mass creation feature to quickly create Sales Records for your Individual Records.

Rx Collect data on Sales Records and Individual Records
Prescriptions
You can add, edit, and remove preferred pharmacies and prescriptions on the beneficiary’s Sales Record and Individual Record.
- Use the Preferred Pharmacies search bar to find pharmacies within 15 miles of the entered ZIP Code. If the pharmacy you're looking for isn't listed, try a different ZIP Code.
- When adding or editing a prescription, you can specify the dosage and indicate whether generic alternatives are acceptable.

Providers
You can add or remove providers on the beneficiary’s Sales Record and Individual Record. Use the Doctors search bar to find doctors, specialists, nurse practitioners, physician assistants (PAs), and health systems within 25 miles of the entered ZIP Code. If the provider you're looking for isn't listed, try a different ZIP Code.

What happens to prescription and provider information on a Sales Record when the record is converted or merged to an Individual Record?
There are Rx and Providers tabs on Individual Records. When a Sales Record is converted or merged, its prescription data will populate in the Rx tab and provider data will populate in the Providers tab.
Can I mass-send Rx Collect forms to consumers?
Yes, via Sales Workflows.
How long until an Rx Collect form link is disabled?
The beneficiary has 48 hours to complete the Rx Collect form after it has been sent. After this period, the link will expire, and they will receive an error message if they attempt to access it through their email.
Will I be notified when an Rx Collect form is submitted?
Yes, the assigned or servicing agent on the record will receive an email notification when the form is completed. No additional setup is required.
