This article explains what the beneficiary sees when completing the Rx Collect portal. It is intended for internal use by AMS+ users only.
Note: Do not share this article with beneficiaries. It is designed to help AMS+ users preview and understand the customer-facing experience.

Cover Page
The portal opens with your agency’s name and logo, along with an optional welcome message. Those can be customized in My Agency Settings. Learn more.
When the beneficiary selects the Get Started button, a one-time passcode is sent to their email address or phone number.
The passcode must be entered on a subsequent Verify Your Identity page and is valid for 5 minutes. If expired, the beneficiary must reopen the link to generate a new code.
Consent Page
This page explains how the beneficiary’s information will be collected and used and includes a required checkbox for acknowledgment. The beneficiary can only proceed after checking the checkbox.
Confirm Your Profile Information
This section shows the beneficiary’s personal information and allows them to make updates. The fields shown—and whether they are marked as required—depend on the section’s configuration in My Agency Settings. Learn more.
If the beneficiary makes changes in this section and submits the portal, the updated information is automatically saved to the record in AMS+.
Disclosures
If enabled, this section shows any agency-specific disclosures configured in My Agency Settings. Learn more.
If an SOA form is included, the beneficiary will see the standard CMS-compliant text and acknowledgment field.
If the beneficiary already completed an SOA within the last 30 days, they will see a message stating that the submission is complete. If required SOA fields—such as the agent’s phone number, signature, or sales rep ID—are missing, the SOA will not appear.
Schedule An Appointment
If enabled, this section displays the Rx Collect portal’s embedded Appointment Scheduler. Learn more. The beneficiary can use it to select a time and date to meet with the assigned agent.
If the beneficiary already scheduled an appointment within the past 30 days, this section will not appear in any new Rx Collect portals sent to them during that period.
Your Prescription Information
This section allows the beneficiary to search for and add their prescription medications. They can enter the drug name, dosage, quantity, and indicate whether a generic alternative is acceptable.
The pharmacy search tool allows them to enter a ZIP Code and find preferred pharmacies within a 15-mile radius. If the beneficiary can’t find their pharmacy, they may need to try a nearby ZIP Code.
Provider Information
In this section, the beneficiary can search for and add their providers (up to 10). Supported provider types include doctors, specialists, nurse practitioners, physician assistants, and health systems.
Search results are based on a 25-mile radius from the ZIP Code entered. If the provider does not appear, the beneficiary can try another nearby ZIP Code.
Confirmation Page
Upon completion and submission, the Confirmation Page appears. It confirms that the form was submitted successfully and includes a customizable message configured in My Agency Settings. Learn more.
In addition, a confirmation email is also sent to the beneficiary.
