This article explains how to enable and configure the Disclosures section in the Rx Collect portal, including how to optionally include a Scope of Appointment (SOA) form.

Requirements
- Your agency’s AMS+ account must have the Accelerate or Elevate subscription package.
- You must be logged into AMS+ as an Agency User.
- You must belong to a Security Group with the Manage Users and Sales Enablement Visible permissions.
- To include a Scope of Appointment (SOA) form:
- Scope of Appointments must be enabled in My Agency Settings. Learn more.
- Each agent must add their phone number, signature, and sales rep ID in their My Account > SOA/ACA Consent tab. Learn more.
About the Disclosures section
The Disclosures section displays required legal content and optional SOA language. Use this section to ensure beneficiaries see important information before continuing the form.
You can customize the following elements:
- Custom Agreement Terms Title
- Custom Agreement Terms
- SOA (Scope of Appointment)
Note: The SOA language is fixed and can’t be edited. However, you may choose to disable and hide the SOA if you do not wish to include it.

How to enable and configure the Disclosures section
- Go to My Agency Settings.
- Find and open the Rx Collect setting.
- In the Disclosures section, make sure the section is enabled (the button should show 'Disable').
This section is enabled by default.
- Select the Edit link next to the "Text: I authorize…" line item.
Update the agency name and terms to reflect your agency's branding.
- To include an SOA, select the Edit link next to the SOA Configuration line item.
- (Optional) In the SOA Configuration pop-up, add an agent/user to the Agents if Record is Unassigned field.
This ensures the SOA has valid agent info when sending requests for unassigned records. If no agent/user is selected, AMS+ will show an error when sending the portal link.
- Set the Initial Method of Contact.
- Add the Plan(s) the Licensed Sales Representative will Represent During the Meeting.
- Select the UPDATE button to save changes.
When editing the SOA configuration, if you add an agent to the Agents if Record is Unassigned field, the SOA will use that agent’s information for unassigned records when Rx Collect is enabled. If no agent is selected, an error message appears when you attempt to send an Rx Collect request and portal link for an unassigned record.
